Launching a Care Integration Resource Center: A toolkit to help tackle medical neighborhood integration complexities

The UCSF Center for Excellence in Primary Care of the Department of Family and Community Medicine has partnered with the Center for Care Innovations to develop an online care integration resource center to better aid primary care providers, care managers, senior clinic leadership, and other health professionals – as well as patients – to help tackle a vital effort to knit together a network that provides coordinated, comprehensive primary care. Created with support from the Blue Shield of California Foundation, the purpose of this Care Integration site is to disseminate strategies, tools, and best practices for integrating primary care with other components of the medical neighborhood.
