I am very pleased to announce the establishment of the UCSF Office of Population Health and Accountable Care (OPHAC). As part of the 5 year strategic plan, “UCSF Vision 2020,” UCSF Health has committed to promoting a high-value system of care by engaging with health care partners to form an accountable care organization (ACO) model. The mission of our OPHAC team is to lead and facilitate implementation of population-based at-risk contracts, develop the internal infrastructure and the external relationships to successfully manage ACO and other risk-based contracts and to provide high value care to defined populations of patients.
The senior leadership team for OPHAC is comprised of Kevin Grumbach, MD, Chair of Family and Community Medicine and Vice President for Population Health, Gina Intinarelli, RN PhD, Executive Director and Ami Parekh, MD JD, Executive Medical Director for Population Health and Accountable Care.
