The SFGH Skilled Nursing Facility at San Francisco General Hospital provides skilled nursing and rehabilitation care in an outpatient unit. The most common indications for care at the SNF include continuation of IV antibiotics, wound care, and short-term rehabilitation. The staffs’ deeper mission is to provide a holistic and interdisciplinary model of care, addressing our patients’ needs for healing and recovery from a bio-psychosocial and spiritual level. We try to maintain an atmosphere of acceptance, community, compassion and encouragement. Through regular contact with caring staff from multiple disciplines – family practice, nursing, rehabilitation, nutrition, social work, and activities – SNF patients return to a level of independent function. Many patients regain a sense of community, self-respect and purpose.
